Happy Holidays from the Leeds & Grenville Community Family Health Team (LGCFHT)!

Our office will be closing at 2:00 pm on New Year’s Eve (Dec 31st). New Year's Day (Jan 1st) the office will be closed.

If you require medical assistance, please call Telehealth at 1-866-797-0000 to speak with a Nurse. If this is a medical emergency, please go directly to your nearest Hospital Emergency Department.

Dental Care for Low-Income Seniors

Learn how to access free, routine dental care for eligible seniors 65 years or older, through the Ontario Seniors Dental Care Program.  This program is designed to support low-income seniors.

To qualify for this program:

Seniors must be 65 years or older, live in Ontario, and have no other form of dental benefits.
