Clinical Manager - Permanent, Part time

The Leeds & Grenville Community Family Health Team (LGCFHT) is seeking a dynamic individual to join our collaborative primary care team of Physicians, Nurse Practitioners and Integrated Health Professionals.

The Clinical Manager, reporting to the Executive Director, will be responsible for clinical leadership, program development and implementation to CFHT Integrated Health Professionals.


  • An experienced clinical leader with a strong background in community/and or primary care

  • A University degree in a relevant health profession

  • Minimum of 3-5 years’ experience, or equivalent combination of education, training and experience

  • Demonstrated Clinical Management skills, including problem solving, critical thinking, conflict resolution and negotiation

  • Demonstrated abilities in program development and the evaluation of outcome measures;

  • Knowledge and proficiency in current, evidenced-based methods and practices of primary care delivery, with an emphasis on health promotion and disease management

  • Experience in working with people from diverse backgrounds and a demonstrated understanding of cultural awareness and sensitivity

  • Strong team player.

  • Experience and knowledge of EMR

  • Strong verbal and written communication skills; a succinct and accurate communicator with excellent interpersonal skills
